Passing the USMLE is a prerequisite for licensure to practice medicine in the US, administered jointly by the Federation of State Medical Boards (FSMB) and the National Board of Medical Examiners (NBME).

USMLE Sample Questions: Types & Examples

Step 1

One of the most common formats for step 1 is a Single-Best-Answer Question. It is linked to a single question with four or more response possibilities (for example, A, B, C, D, and E). You can go through the USMLE Step 1 practice questions to gain a better understanding of the exam.

Several questions require you to interpret graphic or graphical materials. You must choose the best response to the question. Other possibilities may be partially correct, but only one best solution exists. This is the most common and standard multiple-choice question structure on the exam.

Step 2

For Step 2, these are the most common formats:

  • Patient scenarios can be presented in either a Vignette (paragraph) or Chart/Tabular format for any Single Item or Sequential Item Set. The chart/tabular form of test items is intended to mirror patient charts but not to be an exact copy of a patient chart.
  • A patient-centered vignette is linked to a single question with four or more response possibilities.
  • A sequential item set can be linked to two or three questions about the information delivered. Each question is related to the first patient scenario but tests a different point.
  • The abstract item format comprises a summary of an experiment or clinical inquiry provided in a way that a physician is likely to encounter, such as an abstract accompanying a research report in a medical journal.

Step 3

Patient scenarios, pharmaceutical commercials, and abstracts are among the multiple-choice question forms in Step 3.

Computer-based case simulations are interactive simulations of a patient-care situation designed to assess your clinical management approach, including diagnosis, treatment, and monitoring.

USMLE Step 1 Sample Questions

Here are some USMLE  step 1 practice questions to help you prepare well for the exam:

A 27-year-old lady visits the office for preconception counseling. She claims that a friend recently gave birth to a baby with a neural tube defect and wants to reduce her chances of having a child with this issue. She has no history of severe sickness and does not take any drugs. A physical examination reveals no anomalies. It is most acceptable to advise this patient to start taking a vitamin that is a cofactor in which of the following processes?

(A) Biosynthesis of nucleotides

(B) Protein gamma glutamate carboxylation

(C) Scavenging of free radicals

(D) Transketolation

(E) Triglyceride lipolysis

(Answer: A)

Sample Question From Step 2

A 10-year-old kid is brought in for a follow-up exam two days after being treated in the emergency room for hives, hoarseness, and light-headedness. His symptoms started 15 minutes after he was stung by a bee and lasted around 60 minutes before he was treated. He'd been stung by bees thrice in the last year, and his reaction was more severe each time. There are no abnormalities discovered throughout the examination. Which recommendations are most appropriate for preventing future morbidity and mortality from this condition?

(A) Avoid areas known to have bees

(B) Avoid wearing colorful clothing outside

(C) Carrying diphenhydramine tablets

(D) Carrying self-injectable epinephrine

(E) Get medical help right away if you get stung again.

(Answer: D)

Sample Question From Step 3

A 24-year-old man and a 22-year-old lady arrive for the first time at the office for premarital evaluation and counseling. Because they "want to wait" until after marriage to have sexual intercourse, neither of them has ever been sexually active. Other conversations demonstrate that they had never received blood transfusions, taken illegal drugs, or consumed excessive amounts of alcohol. They've heard about the HIV antibody test and wonder if they should get it before getting married. Which of the following statements best describes this relationship in comparison to those who have several risk factors for HIV?

(A) The negative predictive value of the test would be lower

(B) The positive predictive value of the test would be lower

(C) The sensitivity of the test would be higher

(D) The sensitivity of the test would be lower

(E) The specificity of the test would be lower

(Answer: B)

Best Tips to Answer USMLE Questions

Here are a few quick tips to implement while practicing USMLE sample questions that will help you come through the USMLE exam successfully.

USMLE Exam Step 1

  • Try to think of the best answer possible, and then look for it in the list of response options.
  • Choose the most appropriate option from the remaining selections.
  • Carefully read each option and eliminate those that are clearly erroneous.

USMLE Exam STEP 2

  • Practice as much as possible. USMLE practice questions assist you in identifying areas of weakness and focusing on them. They can also help you become acquainted with the exam's format and schedule.
  • Read each question thoroughly. It is critical to comprehend the question.
  • If you're confused about an answer, it's best to guess because unanswered questions are automatically counted as incorrect responses.

USMLE Exam STEP 3

  • The term item refers to a test question in any format.
  • Test questions typically address specific clinical issues from the patient's point of view. One or more pictorials or audio files may be used to supplement the presentation. Many questions from Step 3 require you to evaluate the patient's position in the context of their surroundings, family, or therapeutic setting.
  • You may answer the questions in any order during the time allocated to complete the test questions in a block, examine responses, and alter answers. There is no additional evaluation of questions or change of answers within a block after skipping it.
  • When you take the exam, an online reference will be available with a table of normal laboratory values for frequently ordered laboratory tests, including typical international conversions.
